Reliable support, agreed around the person.
We listen to the older person and their representatives, understand established routines and confirm what has been authorised before service begins. Support is provided respectfully, with clear communication and documentation.
Support may include
- Companionship and supervision in the person’s home
- Support with established daily routines
- Meal, personal-care and mobility assistance as approved
- Meaningful activities based on interests and ability
- Short planned periods of support for family carers
- Clear arrival, handover and progress documentation
- Prompt escalation of health, wellbeing or safety concerns
The exact service, frequency and tasks depend on the person’s assessed needs, approved Support at Home care plan, available budget, Trilogy Care arrangement, location and CCTS service capacity.
