We begin by understanding the participant’s goals, preferences, routines, communication and risks. Supports are then agreed in writing, delivered by suitably matched workers and reviewed as needs change.
Support may include
- Understanding plan budgets and goals
- Connecting with suitable providers
- Coordinating services, meetings and reports
- Monitoring plan implementation
- Preparing evidence for reassessment and responding to change
Availability and funding eligibility depend on individual circumstances, location, service capacity and the participant’s current plan or private arrangement.
