Reliable support, agreed around the person.

We listen to the older person and their representatives, understand established routines and confirm what has been authorised before service begins. Support is provided respectfully, with clear communication and documentation.

Support may include

• Friendly companionship and conversation
• Support to visit family, friends and community groups
• Participation in hobbies, classes and local activities
• Accompaniment to cafés, libraries and community venues
• Support with correspondence and simple technology tasks
• Encouragement to maintain routines, confidence and connection
• Individualised outings based on the person’s interests

The exact service, frequency and tasks depend on the person’s assessed needs, approved Support at Home care plan, available budget, Trilogy Care arrangement, location and CCTS service capacity.
