We begin by understanding the participant’s goals, preferences, routines, communication and risks. Supports are then agreed in writing, delivered by suitably matched workers and reviewed as needs change.

Support may include

• Health and allied-health appointments
• Shopping and essential errands
• Planning routes and appointments
• Support during agreed community activities
• Kilometres and costs documented under the service agreement

Availability and funding eligibility depend on individual circumstances, location, service capacity and the participant’s current plan or private arrangement.
